摘 要
随着社会转型加速，青少年面临的心理压力日益增大，社区成为开展心理健康服务的重要场域。本研究旨在探讨社会工作介入社区青少年心理健康服务的有效模式，以提升服务效能。基于社会生态系统理论和优势视角理论，选取某市三个典型社区为研究对象，运用文献分析、问卷调查与访谈相结合的方法，对325名青少年及其家长进行调研，并跟踪记录18个月的社会工作介入过程。研究发现，通过建立“社工+志愿者”联动机制，构建多层级支持网络，能有效改善青少年心理健康状况；家庭系统干预在预防和缓解心理问题方面作用显著；学校、家庭与社区三方协同合作是关键路径。创新之处在于提出“预防 - 干预 - 跟踪”的全程服务体系框架，强调从个体、家庭、社区三个维度综合施策，形成具有可操作性的实践模式。

Abstract
With the acceleration of social transformation, the psychological pressure faced by teenagers is increasing, and the community has become an important field for developing mental health services. This study aimed to explore the effective model of social work involvement in community adolescent mental health services to improve service efficiency. Based on the theory of social ecosystem and the perspective of advantages, three typical communities in a city were selected as research ob jects, and 325 teenagers and their parents were investigated, and the social work intervention process of 18 months was tracked and recorded. The study shows that the linkage mechanism of "social worker + volunteer" and multi-level support network can effectively improve the mental health of adolescents; family system intervention is effective in preventing and alleviating psychological problems; the cooperation between school, family and community is the key path. The innovation lies in the proposal of the whole-process service system fr amework of "prevention-intervention-tracking", emphasizing the comprehensive measures from the three dimensions of individual, family and community to form an operable practice mode.
