摘 要
随着我国城市化进程的加快和人口老龄化的加剧，社区健康服务面临诸多挑战，社会工作凭借其专业优势在推动社区健康服务方面具有不可替代的作用。本研究旨在探讨社会工作对社区健康服务的推动作用，以期为完善社区健康服务体系提供理论支持与实践指导。基于文献分析和社会生态系统理论构建研究框架，选取典型社区作为案例，运用参与式观察、深度访谈和问卷调查相结合的方法收集数据。研究表明，社会工作者通过资源整合、需求评估、服务设计与实施等环节有效提升了社区健康服务水平，尤其在促进医养结合、心理健康干预以及特殊群体关怀等方面成效显著。

Abstract
With the acceleration of urbanization and the aggravation of population aging, community health services are facing many challenges, and social work has an irreplaceable role in promoting community health services with its professional advantages. This study aims to explore the promoting role of social work on community health services, in order to provide theoretical support and practical guidance for improving the community health service system. Based on literature analysis and social ecosystem theory, typical communities were selected as cases, and data were collected by combining participatory observation, in-depth interview and questionnaire survey. The research shows that social workers have effectively improved the level of community health services through resource integration, demand assessment, service design and implementation, especially in promoting the combination of medical and nursing care, mental health intervention and care for special groups.
